Do K-1 Fiancé Visa Holders Need an Immigration Medical Exam?
Quick Answer
K-1 fiancé visa holders undergo a panel physician exam overseas before entering the United States. However, after marrying their U.S. citizen petitioner and filing Form I-485 to adjust status, they must also complete a domestic immigration medical exam with a USCIS-designated civil surgeon and submit Form I-693.
The Two-Exam Requirement for K-1 Visa Holders
K-1 fiancé visa applicants must complete a medical examination with a U.S. Department of State-designated panel physician at a U.S. embassy or consulate abroad before their visa is issued. This overseas exam uses Form DS-2053 and is required for the K-1 visa itself.
However, once a K-1 holder enters the United States, marries their U.S. citizen petitioner within 90 days, and files Form I-485 to adjust status to lawful permanent resident, a second medical examination is required. This domestic exam must be performed by a USCIS-designated civil surgeon and results in Form I-693.
Why Is a Second Exam Required?
The panel physician exam and the civil surgeon exam serve different legal purposes. The panel physician exam satisfies the medical requirements for visa issuance by the Department of State. The civil surgeon exam satisfies the separate medical requirements for adjustment of status under USCIS jurisdiction.
USCIS does not accept the overseas DS-2053 in place of Form I-693 for adjustment of status purposes. Even if your panel physician exam was recent, you must complete a new examination with a USCIS-designated civil surgeon when filing I-485.
What Is Different About the Civil Surgeon Exam?
The civil surgeon exam follows the same general structure as the panel physician exam — physical examination, TB testing, laboratory tests, and vaccination review — but it is governed by USCIS Technical Instructions rather than State Department protocols. The civil surgeon completes Form I-693 (not DS-2053), seals it in an envelope, and provides it to you for submission to USCIS. The vaccination requirements may also differ slightly from what was required for your K-1 visa.
- Performed by a USCIS-designated civil surgeon (not a panel physician)
- Results in Form I-693 (not DS-2053)
- Required for I-485 adjustment of status
- Follows USCIS Technical Instructions
- Vaccination requirements may differ from K-1 visa exam
When Should K-1 Holders Schedule Their Civil Surgeon Exam?
K-1 visa holders should schedule their civil surgeon exam after marrying their U.S. citizen petitioner and when they are ready to file Form I-485. The I-693 must be signed by the civil surgeon no more than 60 days before submission to USCIS and remains valid for two years from the signature date. Given the 90-day marriage requirement for K-1 holders, many applicants schedule the civil surgeon exam shortly after the wedding to ensure timely I-485 filing.
