Form I-693 Is Valid Nationwide

Form I-693 is a federal document completed by a USCIS-designated civil surgeon. The civil surgeon designation is national — civil surgeons are designated by USCIS to perform immigration medical examinations for applicants anywhere in the United States.

A completed Form I-693 does not become invalid because the client moves to a different state. The form remains valid for 2 years from the date of the civil surgeon's signature, regardless of the client's current state of residence.

No Need for a New Exam Due to a Move

If a client has already completed their immigration medical examination and received a sealed Form I-693, they do not need to repeat the examination simply because they moved to a different state.

The sealed Form I-693 can be submitted to USCIS regardless of which state the client is currently living in. USCIS adjudicates I-485 applications based on the applicant's current USCIS field office jurisdiction, but the medical examination can be completed anywhere in the United States.

When a New Exam IS Required

A new immigration medical examination is required in the following circumstances, regardless of whether the client has moved:

• The Form I-693 has expired (more than 2 years since the civil surgeon's signature)
• USCIS issues an RFE requesting a new or updated Form I-693
• The civil surgeon who completed the original form is no longer USCIS-designated
• The original form used an outdated edition that is no longer accepted
• The original form was lost or the sealed envelope was opened

Choosing a Civil Surgeon After a Move

If a client moves to a different state and needs a new immigration medical examination — for any of the reasons listed above — they should use a USCIS-designated civil surgeon in their new location.
