Preparation Checklist

What to Bring for Your Family

Organized documentation is the single most important factor in keeping a family appointment running smoothly. Use this checklist before your visit.

For Every Family Member

  • All available vaccination records (official immunization cards, provider printouts, or digital records)
  • Government-issued photo ID (passport preferred) — or birth certificate for children without ID
  • Records of any chronic medical conditions or prior surgeries
  • List of current medications with dosages
  • Records of any prior positive TB test (TST or IGRA) and treatment history
  • Payment for each person's exam (cash, credit, or debit accepted)

For Children (Under 18)

  • Birth certificate (required if no government-issued photo ID)
  • Parent or legal guardian must be present throughout the exam
  • All vaccination records including birth hospital vaccines and well-child visit records
  • School health records if available
  • Comfort item for young children (stuffed animal, small toy) to ease anxiety

For Seniors (65+)

  • Complete and current medication list with dosages and prescribing physician
  • Documentation of all chronic conditions (diabetes, hypertension, heart disease, COPD, etc.)
  • Any history of tuberculosis exposure or treatment
  • Records of prior pneumococcal or shingles vaccinations
  • Mobility accommodation needs — notify our office when scheduling

Missing vaccination records? Do not cancel your appointment. Come in with whatever you have. Our civil surgeon can work with incomplete records, advise on what is missing, and administer catch-up vaccines at the clinic if necessary. Contact your primary care provider or our office before the appointment for guidance.

How It Works

Family Appointment Process

From scheduling to sealed Form I-693 in hand — here is exactly what to expect.

Call to Schedule

Call 714-760-4538 and tell us the number of family members and their ages. We block adequate time for everyone and confirm vaccine availability. Recommend 3–5 business days advance notice for families.

Gather Documents

Collect vaccination records, IDs (or birth certificates for children), and medical records for every family member. Organize each person's documents in a separate labeled folder.

Arrive Together

Arrive 10–15 minutes early. Check in as a family. Each person is called individually for their private examination — the civil surgeon evaluates each family member separately.

Exams Completed

Each family member receives a thorough, age-appropriate exam. Catch-up vaccines and required lab tests are administered on-site. Each exam takes approximately 30–60 minutes.

Sealed Form I-693

Each family member receives their own individually sealed Form I-693 envelope. Standard processing: 2–3 business days. Expedited available. Do not open the sealed envelope.
